DISCLOSURE OF PERSONAL INFORMATION AND CONSENT TO PROCESSING SUCH INFORMATION FOR PURPOSES OF ANIMAL RECOVERY
Thank you for choosing Five Star ID as a partner in the protection of your loved animals. To register your RFID microchip we will require you to disclose certain personal information. With reference to section 18 of the Protection of Personal Information Act 4 of 2013, we would like to ensure you that your personal information will be protected and only used for the intended purposes as set out below.
What information is being collected and processed?
The following personal information may be collected through this RFID microchip registration process: your title, full names, surname, contact numbers, e-mail addresses, and physical address. We will also require the personal information of your animal: the animal’s name, type of animal, sex, breed, date of birth, colour, date chipped.
The above information will be requested and obtained from you directly, or from any other third party/ whom you have authorised to submit to us on your behalf.

Why will the information be processed and what is the purpose thereof?
Your personal information will enable us to reunite you with your lost animal. It will be made known if your lost animal is found, and their chip is scanned using the applicable scanning devices.
Is the information given voluntary?
Yes
What are the consequences of failure to provide information?
We will be unable to register your RFID microchip and will therefore be unable to reunite you with your lost animal.
How is the information being processed?
Your personal information is uploaded onto our central Database and IT System where it is stored and used to provide the intended service of identifying a lost pet/ animal’s owner and reaching out to them so that they may be reunited with one another. The scanning device used by approved veterinarians and animal sheltering agencies, allows for the accessing of your personal data via this central Database. We have taken adequate security measures to ensure the integrity and security of your personal data.
Will the information be given to anyone else?
Your information will become known to veterinarian practices and pet shelter agencies to use the applicable scanning devises to scan the lost animal. It may furthermore be shared with the founder of the animal who brought the animal in to be assessed and scanned.
Your rights
You have the right of access to and the right to rectify the information collected. You have the right to object to the processing of personal information as referred to in section 11 (3) of Protection of Personal Information Act, 4 of 2013. You have the right to lodge a complaint to the Information Regulator.
